V3 Sectional Matrix System

Predictably tight contacts, perfect anatomy, and reliable accuracy were key factors in the design.

The V3 Ring, launched in May 2008, was the next generation ring following the very successful V-Ring. The V-Ring brought to Class II restorations two major innovations: V-shaped tines and a nickel-titanium spring.

The shape of the tines solved a longstanding clinical problem: how to get compatibility between the retainer ring and the wedge in the interproximal space. With the V-shape, the ring easily accommodated the wedge. At the same time, the extra width of the tines meant the ring suddenly had much wider indications for use, making it effective in situations where other rings would collapse into a cavity.

The nickel-titanium (NiTi) spring introduced two important new features to the system: its memory and its remarkable elasticity. NiTi has a “memory,” so with constant flexing it will return to its original position, far more so than stainless steel. This gave the ring the advantage of retaining its shape and, therefore, extending its useful longevity. The NiTi spring also exerted optimal separation force over a wider range of embrasure sizes than rings made from stainless steel. This is because, while stainless steel follows Hooke’s Law of Elasticity (which states that the force created is proportional to the extension of the spring), with NiTi the force curve flattens out the greater the extension.

Triodent researchers have established that the ideal separating force for a retainer ring on the teeth is between 3 kg and 4 kg. In testing, the researchers found that a stainless steel ring similar in design to the new V3 Ring was in this ideal region for only a narrow band; between 6.3 mm and 7.5 mm of extension. The two V3 Rings (Universal for molars and Narrow for premolars) generated 3 kg to 4 kg of force through a combined opening range of 3 mm to 8 mm. So, the clinical significance of the flattening curve is that V3 Rings generate the ideal amount of force over a wide range of embrasure sizes.

The original V-Ring did, however, have some stainless steel, an inner band that incorporated the tines. Wanting to remove stainless steel from the picture entirely, Triodent developed the V3 Ring, a stronger NiTi ring and the first with glass-fiber reinforced plastic tines. The shape of the tines gave the ring even wider indications for use, being designed to match buccal and lingual contours of posterior teeth to prevent the ring from collapsing into preparations with wide proximal boxes, even when a cusp was missing. Great thought went into the V3 Ring shape, making their anatomy and force consistent whether the ring was placed mesially or distally, with an ideal cavity form that saved time spent on finishing.

The V3 Ring was also designed with an 18° angle relative to the occlusal plane, making it easy to stack for MODs and simultaneous multiple restorations. Additionally, its shape includes grooves that make it exceptionally stable in forceps.

A ring does not make a system, however, so in the V3 Matrix and the Wave-Wedge, Triodent completes a trio of system components with a huge array of features.

The V3 Matrix band was designed with anatomy the foremost consideration. It includes a rolled marginal ridge which, when positioned correctly, reduces marginal ridge finishing to a minimum. The S-shaped vertical contour also closely replicates the anatomy of the proximal surface and allows the matrix to slide past the gingival margin rather than buckling on the edge. The band is just 30 µm thick but seems stronger because of the overall 3D shape of the matrix. Greater curvature on the horizontal plane means more of that curvature remains as the matrix wraps around the tooth.

The Wave-Wedge’s wave shape and flexible wings adapt to the interproximal space for a superior seal. The wedge also has a hollow underside that protects the gingival papilla. And that concavity means Wave-Wedges are easily stackable, so they can be inserted effortlessly from both the lingual and buccal side during the same procedure.

Both V3 Matrices and Wave-Wedges have holes designed to fit Triodent’s cross-over Pin-Tweezers, for easy insertion and removal. This makes these tiny components very secure, and highly unlikely they will be dropped at that critical moment. The V3 Matrix also has a tab on the top that acts as a handle. When bent in the Pin-Tweezers, that tab provides a “contra-angle,” making positioning easy.

Triodent recommends that the V3 Sectional Matrix System be used with a rubber dam. The V3 inventor and Trident CEO, Dr. Simon McDonald, places the Wave-Wedge before doing his cavity preparation. In nearly all cases he finds he can then place the V3 Matrix without needing to remove the wedge.

V-Ring Questions


1. Do V-Rings produce tight contacts?
Yes! Experts agree that sectional matrix systems are the surest way to achieve tight contacts since the lateral spring pressure acts like a McKean orthodontic tooth separator1.
A V-Ring is a sectional matrix. The only functional differences between the V-Ring system and other sectional matrices are:

The V-Ring is particularly good at holding the matrix in a full anatomical curve.
The V-Ring traps the matrix on the shoulders of the tooth and prevents it slipping and moving away from the neighbouring contact point
The V-Ring spring is NiTi and is very resilient.
The V-Ring spans wide cavities.
The retention is better.
The tooth separation is better as the lateral force of the spring is greater.
When you think about, it can't help but produce excellent contacts.

2. Can V-Rings be used on wide cavities?
Yes! The anatomically shaped tines grip on both adjacent teeth and this prevents the spring from falling into wide cavity preps (like other sectional matrices).

3. Can V-Rings be used when a cusp is missing?
Yes! Since the tines grip on both teeth either side of the contact and also low down near the gingival margin, a V-Ring can often be used when a cusp is missing. Depending on the anatomy, it is sometimes best to build up the bulk of cusp before placing the V-Ring.


4. Can V-Rings be used on primary teeth?
Absolutely! V-Rings work surprisingly well on primary molars.

Absolutely! They designed for re-use.

6. Can V-Rings be used on MOD's?
V-Rings work really well for single Class II restorations and MOD's. You can stack the V-Rings or have one facing mesially and the other distally.

7. How can the extra cost of V-Rings be justified?
V-Rings are not much more expensive than other competing systems and the produce better results with less frustration and time spent. They are also made of stainless steel and NiTi which lasts longer. The cost of the consumable Tab-Matrix is similar to other sectional matrix bands.


In addition, think how much it costs you every time you have to re-do a proximal restoration because it has a poor contact. If you try to re-do it right away you run late and get stressed out. If you do it later the patient "bad-mouths you" because they think you are incompetent. You loose credibility and wasted more time with more costs and no income when you do have to re-do it! Arrrgh!

8. Can you use V-Rings to restore TWO adjacent Class II cavities?
Yes. The separation that V-Rings generate is 60-80 microns, which is great than two thicknesses of matrix.

9. Do V-Rings work equally well on all teeth?
The V-Ring is a very veratility system and no other matrix system comes close in this respect.

10. How do V-Rings and Tri-Clips compare?
The V-Ring cost more to begin with but less per restoration. It takes a tad longer to place but saves time on finishing and, in my hands, produces the most anatomically correct restoration I have ever done. The V-Ring is more versatile that the Tri-Clip and works well in some really tricky situations. It is also easier to learn how to use. The Tri-Clip costs less to get started and but more per use, and, in my experience, works well on the first molar forward but is harder to get in place further back, particularly in the maxilla.

11. Do I have to hold the contact point with a burnisher while light curing? Could I use a regular matrix and hold the contact and then why would I need a V-Ring?
I always do this because it is so easy to do and avoids the bad contact caused by the matrix not being in contact with the neighbouring tooth. Quite often it is impossible to see if the matrix is actually touching so holding is just a precaution. I reckon that this single issue is the main cause of REALLY bad contacts. The V-Ring's function is to separate the teeth (orthodontically) and wrap the matrix. Just as an asise, I prefer to add a tad more curvature with my fingers before placing the matrix as it makes more room to place the V-Ring.
A regular matrix (Tofflemire) when used on a Class II has the effect of moving the tooth in the wrong direction by the thickness of the matrix (because you have to get the matrix through the intact side). This means that you have to overcome TWO thicknesses of matrix to get a tight contact. Tofflemire also tend to produce cylindrical restorations with a poor contour and a contact just at the marginal ridge. This method has been tried over and over and found to be be unpredictable by most who have tried it.

Tab-Matrix Questions

1. How do I use the Tab?
You simply bend the tab towards you (mesially) and hold it with any kind of tweezers and then move it apically into the embrasure. Try to avoid trapping gingival tissue between the matrix and the gingival margin of the cavity.

While burnishing the Tab-Matrix, lift the tab over and fold it out of the way.

3. How do I remove the Tab-Matrix?

First lift the matrix away from the restoration to un-stick the adhesive. Then use the special TrioDent Pin-Tweezers and grab the hole on the wings of the Tab-Matrix and simply pull. Grabbing the Tab often rips the tab off but the lateral holes work better.
You can also try flossing (waxed floss) between the matrix and the restoration.

In the above image, note how the Tab-Matrix has been peeled away from the restoration so that it can be rotated about the contact point. It will then slide out easily if you have a good grip with either the Pin-Tweezers or hemostats.

Please note that the Tab is ONLY for placement and NOT for removal. We are convinced that the bonding agent glues the matrix to the composite so it is essential to unstick the matrix with a probe or flat plastic instrument rather that keep pulling. Once you have separated the matrix you should be able to wiggle it so that it rotates about the contact point. It then can be wiggled out relatively easily even though you have a very tight contact.

Description

V-RingSectional Matrix System facilitates the contouring of posterior Class II composite restorations. The V-Ring separator has a stainless steel inner ring and a nickel-titanium outer spring with V-shaped tines. The tines are placed over the wedge on the buccal and lingual surfaces to enhance tooth separation. Each sectional matrix band has a tab with a hole on the occlusal edge and holes on the wing ends for pin tweezers to grasp during placement and removal. Matrices are available in three heights: 4.5, 5.5 and 6.5 mm. The system also includes V-Wedge, Double-Wedge and the Wave-Wedge. V-Wedge has an inverted V-shape cross-section to protect the papilla and is available in six different color-coded sizes. The Double-Wedge is a clear elastic wedge that is designed to be stretched and pulled between the teeth from the occlusal aspect, rather than inserted from the buccal or lingual direction like a traditional wedge. The Wave-Wedge has an inverted V-shape cross-section, comes in three color-coded sizes and is a longer wedge to seal both the buccal and the lingual aspects of the matrix tab. The Wave-Wedge’s effect is similar to placing two wedges, one from the buccal and one from the lingual. The kit evaluated contained three V-Rings, 50 Tab-Matrices in each of the three sizes, ring forceps, pin-tweezers, V-Wedges, Double Wedges and Wave-Wedges. Thirteen consultants used the V-RingSectional Matrix System in over 500 clinical restorations. This product received a 96% clinical rating.

Product Features

To supplement an instructional CD/DVD, the V-Ring Sectional Matrix System provides a written photo series showing the correct placement of matrix, wedge of choice and placement of the V-Ring. Improved packaging for the V-Ring Setional Matrix System now consists of a plastic container with individual compartments for each component. The pin holes in the matrix bands aid in grasping the bands during placement and removal. Bands adapt to the teeth well with a good selection of sizes. The most positive feature of the system is the V-Ring component itself. Consultants appreciated the excellent fit to the buccal and lingual embrasures, separation of the teeth, and the subsequent excellent contacts that were formed in the final restoration. The variety of different wedges required some practice to master and some consultants found it easier to focus on their traditional wedges or simply use the V-Wedge. Some consultants preferred the older version of the locking pliers, as opposed to the newer pin tweezers, which are still available through the manufacturer. Consultants felt that a variable height of tines with the V-Ring would be helpful for using multiple V-Rings to restore mesial and distal surfaces simultaneously. Overall, 92% of consultants would switch to the V-RingSectional Matrix System. All consultants found the V-Ring Sectional Matrix System to be equal to or better than their current sectional matrix system and would recommend it to a colleague.

Ideal Separation

One of the unique features of V3 Ring is that the ring itself—not the wedge—creates the interproximal separation. A fully tensioned V3 Ring is capable of producing 60–80µm of interproximal space, making it easy to accommodate the wedge and form the matrix around the tooth.

Long-Lasting Performance

The V3 Ring is designed to provide consistent results over many Class II restorations. Over time, however, the NiTi of the V3 ring can lose a bit of its memory—especially if the ring is left in the forceps too long. If this happens, simply place the V3 Ring in the notches of the forceps handle for 40–60 seconds. This will re-tension the V3 Ring, making it good as new!

Every part of the V3 Sectional Matrix System has been expertly designed to work together, creating a totally seamless procedure. The unique V-shaped tines of the V3 Ring fit precisely over the wedge, and the wedge fits perfectly over papilla.   • Do V3 Rings produce tight contacts?

    Yes. Experts agree that Triodent sectional matrix systems – using the V3 Ring - is the surest way to achieve tight contacts since the lateral spring pressure acts like a McKean orthodontic tooth separator.¹ A V3 Ring IS a sectional matrix. The only functional differences between the V3 Ring system and other sectional matrices are:
    1. The V3 Ring is particularly good at holding the matrix in a full anatomical curve.
    2. The V3 Ring traps the matrix on the shoulders of the tooth and prevents it slipping and moving away from the neighboring contact point.
    3. The V3 Ring spring is made from nickel-titanium (NiTi) and is very resilient.
    4. The V3 Ring spans wide cavities.
    5. The retention is better.
    6. The tooth separation is better as the lateral force of the spring is greater.

    When you think about it, it can't help but produce excellent contacts.

  • Can V3 Rings be used on wide cavities?

    Yes. The anatomically-shaped tines grip on both adjacent teeth and this prevents the spring from falling into wide cavity preps (like other sectional matrices).

  • Can V3 Rings be used when a cusp is missing?

    Yes. Since the tines grip on both teeth on each side of the contact and also low down near the gingival margin, a V3 Ring can often be used when a cusp is missing. Depending on the anatomy, it is sometimes best to build up the bulk of the cusp before placing the V3 Ring.

  • Can V3 Rings be used on primary teeth?

    Yes. The V3 Ring comes in two sizes – Universal (green) for molars and Narrow (yellow) for premolars and primary teeth.

  • What degree of separation is achieved with the V3 Ring?

    The ring will create a separation of 60-80 microns. This is greater than the thickness of two Triodent matrices and is sufficient separation to restore two adjacent Class II cavities at the same time.

  • Can V3 Rings be sterilized and re-used?

    Yes. They are designed for re-use and have been tested successfully through 800 autoclave cycles. The rings should be steam-autoclaved, maximum temperature 135°C. Other methods of sterilization run the risk of damaging or staining the rings. Chemical sterilization will void our warranty.

  • How do I remove material that has adhered to the V3 Rings?

    Do not soak the V3 Rings in alcohol. Wiping the V3 Rings with a soft cloth soaked in warm water or scrubbing with a soft nylon brush under warm water is effective at removing the majority of materials. If the composite materials are too firmly adhered to the V3 Rings to be wiped away, the material can first be softened by soaking the rings in warm water. Soaking may be followed up by ultrasonic cleaning using a mild detergent such as BioSonic® Enzymatic Ultrasonic Cleaner.* Materials can also be gently wedged and lifted using a blunt plastic instrument such as a composite spatula. Abrasive instruments, like metal dental hand instruments, can damage the surface of the tines and are not recommended.  Rings should then be rinsed and steam autoclaved according the IFU.

  • How can the extra cost of V3 Rings be justified?

    V3 Rings are not much more expensive than other competing systems and they produce better results with less frustration and time spent. They are also made of NiTi and glass-fiber reinforced plastic, which lasts longer. The cost of the consumable V3 Tab-Matrix is similar to other sectional matrix bands. In addition, think how much it costs you every time you have to re-do a proximal restoration because it has a poor contact. If you try to re-do it right away you run late and get stressed out. If you do it later the patient “bad-mouths” you because they think you are incompetent. You lose credibility and waste more time with more costs and no income when you do have to re-do it! Arrrgh!

  • Do V3 Rings work equally well on all teeth?

    The V3 Ring is a very versatile system and no other matrix system comes close in this respect.

  • Do I have to hold the contact point with a burnisher while light curing? Could I use a regular matrix and hold the contact and then why would I need a V3 Ring?

    Dr McDonald always does this because it is so easy to do and avoids the bad contact caused by the matrix not being in contact with the neighboring tooth. Quite often it is impossible to see if the matrix is actually touching, so holding is just a precaution. Dr McDonald believes this issue is the single main cause of REALLY bad contacts. The V3 Ring's function is to separate the teeth (orthodontically) and wrap the matrix.

    A regular matrix (Tofflemire) when used on a Class II has the effect of moving the tooth in the wrong direction by the thickness of the matrix (because you have to get the matrix through the intact side). This means that you have to overcome TWO thicknesses of matrix to get a tight contact. Tofflemires also tend to produce cylindrical, “tin can”, restorations with a poor contour and a contact just at the marginal ridge. This method has been found to be unpredictable by most who have tried it.

  • Which style of clamp should I use to hold the rubber dam in place?

    Often there is room for both the clamp and the V3 Ring. Our general advice is to use wingless clamps if space is going to be an issue. For example, on a #30 tooth, try a W3 or W7. Some brands of clamp leave more room than others. Don’t dismiss winged clamps entirely. If possible, place the clamp distal of the tooth being treated. Many dentists also use the Triodent® Wave-Wedge routinely to help keep the dam in place.

  • Does the V3 Ring work if you are doing three teeth in a row i.e. #12DO, #13MOD, and #14MOL?

    Dr McDonald prefers to prep all at once, place the matrices on all teeth then place composite initial layers on all three teeth. Then he finishes layering 12 and 14 and pulls off the matrix on 12 and 14. Next he pushes the wedge in farther between 12-13 and 13-14, burnishes the contact on M and D of 13 and places the final layer on 13. Then he finishes all three at the same time.

  • With the V3 Ring is it best to do them all separately start to finish or would you do 12 and 14 together and then 13 by itself?

    Yes you can do all four in a row if you want. You just need to make sure that the V3 Ring is separating the contact you are working on. You can move the V3 Ring along as you work, or stack rings. Dr McDonald prefers to use just one V3 Ring as it allows that embrasure to open rather than the retainer rings working against each other. Although he builds one contact at a time, he etches, bonds and places bands and/or matrices all at once.

  • The V3 Ring is a small piece of equipment to put in the patient's mouth. What if it springs out or is dropped?

    The V3 Ring is very stable on wet or dry teeth because of its strength and anatomically shaped faces. However, for added security tie floss to the ring.

  • How do I retension the V3 Ring if the tines start to separate?

    We use NiTi in our rings. It has remarkable strength and elasticity, so the ring will return to its original shape again and again. The V3 Ring may separate slightly over time with over-stretching, but can be easily retensioned by placing it in the notches in the handles of the Triodent® Forceps. Squeeze it like a nutcracker until the tines touch and the ring is as good as new. To prevent over-stretching, only open the ring as far as is needed to place it over the tooth. Also, try not to leave the ring open in the forceps for a long period of time.

  • Can V3 Rings be used on MOD's?

    V3 Rings work really well on single Class II restorations and MOD's. You can stack the V3 Rings or have one facing mesially and the other distally. The ring tilts up, away from the marginal ridge of the tooth, allowing more room for another ring to be placed underneath, on the next tooth. In most cases it is advisable to place the rings in opposite directions.

  • What's the best way to restore a back-to-back MO or DO?

    The easiest method for back-to-back Class II restorations is to complete one and then do the other. Doing both simultaneously can be faster but requires more operator skill. A Narrow (yellow) V3 Ring is recommended as more separation is necessary to overcome two thicknesses of matrix.

  • What is the best way to restore an MOD with the V3 Ring?

    The recommended method is as follows:
    1. Pre-wedge both M and D and complete cavity preparation.
    2. Slide M and D matrix into position and etch and bond.
    3. Place V3 Rin​g on M and build and cure M contact point.
    4. Move V3 Ring to D and build and cure D contact point.
    5. Complete occlusal surface and cure with C-Factor issues in mind.

  • What is the best way to remove the matrix band when it's in a tight contact area and stuck to the composite?

    Push a thin, metal-blade instrument like a probe through the embrasure below the contact point to break the bond. Some clinicians also recommend flossing between the restoration and the band.

  • Can I place the Wave-Wedge with the V3 Ring in place?

    Yes. The V-shaped tines of the V3 Ring allow you to place a wedge without removing the ring. And because the Triodent Wave-Wedge has a hollow underside, you can slide a second wedge from the opposite side to the first.

  • When doing an MOD, the matrix band side extrusions are too long and get in the way of the opposing matrix band?

    The easy way around this is to place a V3 Ring on the first tooth, trapping the matrix band in place, then bend the protruding ends of the matrix backwards to make room for placement of another band on the opposite side of the restoration.

  • Which style of rubber dam clamp should I use on, for example, a #30, that will not interfere with the V3 Ring?

    Firstly, it is usually better to use a wingless clamp, say a W3 or W7, but there are other options to consider, like an 8a or a serrated premolar clamp. Yet another thing to try is to use a normal clamp, rotated slightly, on the #31.

  • Sometimes when I place the V3 Ring on the SuperCurve matrix, the ring and matrix slip up the tooth. How can I prevent that?

    This is most likely to occur on the buccal of lower molars and lingual of upper molars. If it happens, try placing the ring mesially instead of distally. Otherwise, wedge from both sides with Wave-Wedges or use the hard Triodent®V-Wedge to secure the matrix. It’s a good idea to push down on the ring immediately after placement while it is still in the forceps to make sure the ring is fully seated on the undercuts, and keep your finger firmly against the matrix tab until you have released the forceps to ensure there is no slippage.
